This form has been developed to capture user inputs for the MARS EEI report, and automatically e-mail the results.  The form will take user inputs and format them into an ordered list of categories and values according to the EEI format. The form computes the Date-Time Group, allows selection of the appropriate event identifier, and provides "text blocks" and "radio buttons" in the various categories to formulate the rest of the report.

For a real report, copy or type the DOMS address shown, into the "To:" block.   For testing purposes you can put any e-mail address in the "To:" block.   Suggest you test the form first so that you get a feel for what would be sent.  Multiple "Info:" addresses can be selected from the list provided. In addition, the user will automatically receive an e-mail copy to confirm the report sent.

This form is set up for USAF MARS Region 5 so it has the Region 5 officials listed.  It also has the National Emergency Coordinator, the Assistant NEC and the Chief, USAF MARS.
